Hartford Joint #1 Wellness Program

July 1, 2023 through April 30, 2024

 

Eligibility: All employees are eligible to participate in the District’s wellness program.  Spouses covered by the district’s insurance plan may also participate in the wellness program. The employee and the spouse’s achievements will be considered separately and the spouse’s requirements are the same as the employees.

Wellness Tasks Required:  Documentation of wellness tasks will be required to satisfy requirements. All tasks must be completed by April 30, 2024 and all documentation must be submitted to  Dina Lane by May 15, 2024 to receive credit. Employees that do not complete the tasks as outlined for either the $700 or the $350 payment will not receive a wellness stipend.

All tasks must be completed to receive a full $700 payment. The tasks include:

  1. Confirmation of an annual physical with biometrics;
  2. Participation in a District sponsored activity challenge in October and April (must satisfy requirement for both months);
  3. Documentation of receiving a flu shot (only required if not participating in the District’s sponsored flu clinic) or a written excuse from the employee’s medically certified medical professional or spiritual leader requesting the employee be exempt from the vaccine for medical, personal, or spiritual reasons.

 

A $350 payment (50%) can be earned by completing the following tasks:

  1. Confirmation of an annual physical including biometric;
  2. Completing two (2) out of the following three:
    1. Participation in a District sponsored activity challenge in October;
    2. Participation in a District sponsored activity challenge in April;
    3. Documentation of receiving a flu shot (only required if not participating in the District’s sponsored flu clinic) or a written excuse from the employee’s medically certified medical professional or spiritual leader requesting the employee be exempt from the vaccine for medical, spiritual or personal reasons.

 

Wellness Program Payment:  Employees who participate will be eligible for either contingent payments in the form of HRA contributions (health insurance subscribers only) or stipends (non-health insurance subscribers). Any payment earned by the covered spouse will be made in the form of an HRA contribution. All wellness HRA contributions and stipends will not be made until all tasks are completed and all employee records updated.  The District anticipates making payments between May 15 and June 30.

Proposed Activity Schedule (not in the employee handbook, reference only)

  • October – Steps challenge – month long activity (Employees hired after October 2023 may choose either February 1 - February 28, 2024 OR March 1 – 31, 2024 to do a Steps challenge in place of October.) 
  • Friday, November 10 –Flu Shot Clinic. Open to all HJT#1 employees.  Employees not on District insurance should bring a copy of their health card for billing to their insurance company. Documentation of flu shot administered elsewhere is accepted.
  • December - None
  • April - Steps challenge – month long activity
